Payment Information
The Gynecologic Oncology Group will only accept Visa, MasterCard, Check or Money Order. (Payable to the Gynecologic Oncology Group.)
Registrations/Payments may be sent online, by fax, email or by mail. Please use only one of these methods. Duplicate submissions will result in duplicate registrations and duplicate credit card charges.

PLEASE BE ADVISED: Registrations that require payment will not be processed until full payment has been received in the GOG Administrative Office.
All Mailed or Faxed Registrations will be confirmed within two weeks of Receipt.
Payment Refund/Cancellation Policy:
Full Refunds, less $25.00 Processing Fee per registrant will be granted for cancellations received in writing prior to July 3, 2008. No refunds will be granted after July 3, 2008, but registration substitutions may be made in writing without penalty. No Refunds will be given to registrants who do not attend the GOG meeting, without prior cancellation notice.

Online Registration Instructions:
Each registrant will be asked a series of information (including Birthday) to create a record. The birthday will be used as an identify feature, thus creating an individual profile that can be used for each meeting. After the registrant registers for the meeting, a confirmation will be emailed directly back to the email address provided on the registration form. This information is very important for CME Tracking. All Certificates will be mailed to the address given on the registration form.
GOG Members

Please Note: You must be in the GOG Roster and have a GOG Web Menu Account in order to access online registration as a GOG member.
Non-Members
On the Online Registration Page please click the link for Non-Member Registration. No password is required.
ONLINE PAYMENT
VISA and MasterCard will only be accepted for payments. A confirmation will be given immediately upon submitting your registration and/payment. Please print your registration confirmation.


Committee/Workshop Registration
The Gynecologic Oncology Group now requires that you register for each Committee Meeting that you plan to attend. Please select all committee meetings on the enclosed registration form. The GOG has applied for Continuing Medical Education Credits (CME’s) for most of the meetings listed.
Important CME Information for Committee Members
CME’s can only be obtained when you sign the pink sign-in sheets outside the meeting rooms or at the CME desk. Committee members will not receive CME’s if they have only signed the committee sign-in sheets. They also must sign the CME sign-in sheets (pink).
Semi-Annual Committee/Workshops CME Credits
All PINK sign-in sheets must be signed, and an overall meeting evaluation must be filled out for each committee/workshop that you attended in order to receive CME credits. Sign-in sheets (PINK) are located outside each meeting room. Please visit the IT room to complete the evaluations online.

Please Note: To complete the Online Evaluation Form at any location other than the IT Room you must have Adobe 7.0 Professional.
All Committee/workshop CME Certificates WILL NOT be handed out onsite or in your meeting packets. You will receive your CME Certificate approximately five weeks after the meeting upon completion of an evaluation form and verification of signatures on sign-in sheets.
Symposium CME Credits
A meeting evaluation form MUST be filled out in order to receive your CME credits. Evaluations will be available in the meeting room or the CME evaluation desk. If you have pre-registered they will be in your meeting packets. If you are registering onsite you can go to the CME registration desk to receive your certificate. All certificates for the Symposium will be validated after the completion of the course.
