The GOG Foundation, Inc. (“GOG”) is a District of Columbia not-for-profit corporation with its headquarters and principal place of business in Philadelphia, Pennsylvania. The GOG Foundation, Inc. is a charitable organization exempt from taxation under section 501 (c )(3) of the Internal Revenue Code. The following states require special notices to be included with a solicitation.

Maryland: For the cost of postage and copying, documents and information filed under the Maryland charitable organizations laws can be obtained from the Secretary of State, Charitable Division, State House, Annapolis, MD 21401, 1-800-825-4510. A copy of the organization’s current financial statement is available upon request.
Georgia: The following information will be sent upon request: (A) A full and fair description of the programs and activities of The GOG Foundation, Inc.; and (B) A financial statement or summary which shall be consistent with the financial statement required to be filed with the Secretary of State pursuant to Code Section 43-17-5.

Washington: You may obtain additional financial disclosure information by contacting the Secretary of State at 1-800-332-GIVE.
West Virginia: West Virginia residents may obtain a summary of the registration and financial documents from the Secretary of State, State Capitol, Charleston, WV 25305.
